All clients who participate in Root Cause Repatterning Sessions should be advised of the following information:

Alicia Lynn Diaz is a Certified Ayurvedic Practitioner and is not licensed by the state of California.  There is currently no license available for the services provided.  All services and treatments provided are complementary or alternative to health care services provided by health care practitioners currently licensed by the state of California.

Services Offered:
A consultation will include discussion of the client’s concerns and reason(s) for the visit, past treatment results, short term and long term healthcare goals, current treatment options and client’s expectations.
A consultation will include an Ayurvedic assessment of the client as well as education related to Ayurvedic theory and practice. Recommendations may include information and instruction on dietary modifications, meal preparation, lifestyle adjustments, herbs, yoga, meditation, Marma (therapeutic use of energetic points), and Pranayama (breathing practices). Recommendations provided will be tailored to the unique needs of the client with the intention of assisting the client to achieve better health and a greater sense of well being. Any new practices will be taught and then practiced with the client to ensure client’s understanding and comfort level with the practice. Educational resources will also be provided for the client to refer to as needed. Initial recommendations will be provided during the assessment, and if applicable, more detailed recommendations will be e-mailed to the client.

Notice regarding client records:
A client has the right to access their own records and written information therein.
Client records and transactions are confidential unless the release of these records is authorized in writing by the client or as required by law.
A client has the right to a coordinated transfer when there will be a change in the provider of the complimentary and alternative health care services.

Your agreement:  By accessing this information, you understand there may be limitations to the advice provided by Alicia Lynn's consultation and that in your best interest, according to the professional training and experience of your consultant, you may be referred to another practitioner or healthcare provider.  You understand that the consultant is not providing Western (allopathic) medical care, and that you should look to your primary care doctor for those services and for routine check-ups.  You are also aware that your consultant cannot prescribe, administer or recommend the discontinuation of controlled substances and drugs prescribed by a licensed health care provider.

By accessing this information, you understand that in the tradition of Ayurveda, and other alternative therapies, the integrated treatment of spirit, mind and body is an inherent and core philosophy of health and wellness. For example, marma points, yoga poses, meditation and breathing practices are understood to affect either the physical and/or spiritual, and have been used in this manner for many centuries. There may be times when recommendations from a consult will result in the transformation of physical and emotional energy. These transformations may result in such feelings as sadness, grief, and anger or even joy and a sense of peacefulness. You understand that your consultant’s support during such emotional expressions must not be interpreted as providing counseling in the manner of a licensed counseling professional, and is not a substitute for such care.

By accessing this information, you also understand that the number and frequency of consultations you will require for your condition depends upon many factors, including, but not limited to, the severity and/or chronic nature of your complaint, and how well you are able to incorporate the consultant’s recommendations and make lifestyle changes.  You understand that you and your consultant will discuss the number and frequency of consultations that you will require. You also understand that the number and frequency of consultations you require may vary throughout your course of healing.

Cancellation Policy:
Should you need to cancel your appointment, please provide at least 48 hours notice. This allows time to give your appointment to someone else in need.
